To receive your transcript, fax or mail a completed copy of the Transcript Request form to the SNU Registrar's Office (as listed below).
In order for us to process your request, please be sure to include the following information:
- Date of the request
- Your last year of enrollment
- Your full legal name
- Your social security number
- The number of copies needed
- Your current address
- Address(es) of where you want the transcript(s) sent
- Your handwritten signature
- A phone number where we can contact you if there are any problems
There is no charge for transcripts. However, if you have any unpaid accounts with the university, transcripts will not be issued until clearance is received from the Business Office.
Tulsa students may come to the Tulsa Campus and complete a transcript request form that will be faxed from our office to the Bethany Registrar.
We respond to faxed and mailed transcript requests in 3 to 5 business days.
